Sliding Fee Discount Program
The Sliding Fee Discount Program (SFDP) is a
Federal Program that allows Big Sandy Health
Care, Inc. to discount our health care services.
The SFDP program is available to all patients
who qualify based on family size and income,
regardless of insurance status. If you do not
have insurance or you struggle to pay copays,
our sliding fee discount program may help.
How to Apply:
To apply you will need to complete the SFDP
Program application and bring in proof of annual
income for all family members in your household.
Documents accepted for proof of income
are one of the following:
- Employment checks stubs for the past two months
- Income tax return (for self employed individuals)
- Bank statements showing direct deposit amounts
- Other legitimate official documents
- Statement from Division of Unemployment Services
- Statement from employer on company letterhead stating wages
- Statement of income determination from the
- Department of Housing
- Social Security/Disability award letter
